In my experience, we prioritize actions based on RPN (highest to lowest). Requesting you to provide clarity on why option A is incorrect.

Hi Rajesh,
this has to do with safety and thus the first step is to consider the highest severity items, not necessarily the highest RPN value. If using a 10 point severity scoring system, the first step is to address the 9 and 10 severity scores first, then move on to the highest RPN values. Safety first
