Primer On Pipeline Integrity Management

Primer On Pipeline Integrity Management

A few weeks ago I met with Cesar de Leon who is a pipeline safety expert (http://www.pipelinesafetyconsultant.com) – he worked at the DOT Office of Pipeline Safety for over 23 years and currently resides in Boerne, Texas. Cesar was telling me about legal and technical issues arising in the field of natural gas pipelines from existing regulations… I will discuss these issues in greater details in latter posts. Before getting into current issues, I would like you to read the following guest post from Cesar that provides a brief overview of pipeline integrity management plan.

Could a ‘No’ as any other Word be a Positive?

Could a ‘No’ as any other Word be a Positive?

What’s in a word?

The right word at the right time can make all the difference between a successful negotiation with an amicable agreement and collaborative banter, or a bitter and twisted tête-à-tête with an entrenched stand-off, raised hackles and, at best, fog-horn diplomacy.  But what is the ‘right word’; unfortunately, it depends on the time and place and people’s personality but one thing is almost certain, if it begins with “n” it is oftentimes not considered to be right and many consider it to be downright wrong.

Will Your Refresher Training Work?

Will Your Refresher Training Work?

In the last two years, I have traveled significantly…have an elite status on three airlines and lots of frequent flyer miles. There are quite a few aspects of a trip that are routine and boring. One such thing is the safety announcement or video at the beginning of each flight. Why do they have it?

Learning Curves: A Bitter buy Undeniable Risk!

Learning Curves: A Bitter buy Undeniable Risk!

Guest Post by Malcolm Peart (first posted on CERM ® RISK INSIGHTS – reposted here with permission)

Despite our collective educational establishments that purport to teach so that people can learn, ‘education’ is not necessarily learning.  Mark Twain wrote disparagingly that “Education consists mainly in what we have unlearned” and also that he never let schooling interfere with his own education.  Education is not necessarily knowledge and real learning comes from the application of theory tempered with experience which will make for better decisions and better outcomes.  Benjamin Franklin’s words from over 200 years’ ago “Tell me, I forget. Teach me, I remember. Involve me and I learn” still ring true today.

GAO: CSB Not Meeting Its Statutory Mandates

GAO: CSB Not Meeting Its Statutory Mandates

According to a recent report (Aug. 2008) from the Government Accountability Office (GAO), the Chemical Safety Board (CSB) is not in compliance with its statutory mandates.  The CSB is an independent federal agency charged with investigating industrial chemical accidents and has been in operation since 1998. GAO report states that CSB is not investigating all chemical releases that have a fatality, serious injury, substantial property damage, or the potential for a fatality, serious injury, or substantial property damage. Serviceability

Serviceability

The technician explained it to me like this: “The part costs only $5, but I need to disassemble other stuff to get to it, so my labor charge will be $300. But don’t worry, I replace lots of these.”

Anticipatory Leaders Leverage Future Facts (TM)

Anticipatory Leaders Leverage Future Facts (TM)

Guest Post by Daniel Burrus (first posted on CERM ® RISK INSIGHTS – reposted here with permission)

We cannot turn back the clocks and undo the damage the pandemic has done, but we can move forward with an anticipatory mindset and a firm plan of action. For many decades now, and in all industries, digital disruption has been on an exponential curve upward. Because the pandemic forced us to turn to technology for help on both a personal and business level, the pace of technology-driven change increased dramatically, far beyond merely exponential levels. Now that this change has been set in motion, the question remains: What do we do now?

Is Your Organization Making The Right Decisions?

Is Your Organization Making The Right Decisions?

Eating a few fries this one time will not increase risks of a heart attack. Plus I like the fries from this joint. I’m ready to order!!

That was a relatively easy decision; however, there are certain decisions that are more complex and may have significant implications to your organization’s future including organizational safety.

Victoria’s New Risk Management Framework

Victoria’s New Risk Management Framework

Guest Post by James Kline (first posted on CERM ® RISK INSIGHTS – reposted here with permission)

Enterprise Risk Management (ERM) is considered an important management tool and part of good public sector governance in Australia. The Commonwealth of Australia and the states of Victoria and New South Wales have implemented ERM. In addition, the states of Tasmania and Western Australia have issued good governance frameworks which include risk management.  This piece will provide a historical overview of the risk management approach used by the Australian State of Victoria. It will then discuss the changes made in the 2020 revisions to its 2018 Victorian Government Risk Management Framework.

Developing Trust While Communicating Risks

Communicating risks to public from chemical or nuclear facilities is a major challenge. We notice this over and over again with EPA community-right-to-know, nuclear power plants and lately with the LNG facilities.

Career Long Lessons From a Once in a Lifetime Disruption

Career Long Lessons From a Once in a Lifetime Disruption

Guest Post by Daniel Burrus (first posted on CERM ® RISK INSIGHTS – reposted here with permission)

For some, the post-coronavirus “new normal” is a frightening concept that rattles their status quo, while others are taking this moment to seize new opportunities afforded to us by a global disruption, as COVID-19 has proven to be. The fact of the matter is that aforementioned opportunity is available to all of us, whether our “new normal” is unbearably difficult or extremely simple.

You may be asking yourself: How is that possible?

Maintaining Process Safety During Tough Economy

Maintaining Process Safety During Tough Economy

We appear to be in a global recession – companies all over are struggling to deal with impact of economic slowdown. In an attempt to maintain profits, companies are drastically reducing their spending and also trimming down their workforce. It is anticipated that one-fourth of the employers in the US will undergo workforce reduction in 2009. As a result of pessimism and uncertainty about future, there is an increased level of anxiety amongst the employees. There are a few issues that a chemical manufacturing firm must address to ensure safe operation during the downturn:
