As long as humans are involved in running our organizations, there will be holes (vulnerabilities) in our cheese. This is why I state that we will never have American cheese (no holes) representing our safety systems, because this would imply they would be failsafe and not impenetrable.

No matter where we work, we will experience failures or ‘undesirable outcomes’ of some kind. As long as we work with other humans, this will indeed be the case. In an era of rapidly advancing technology, the need for training to keep up is imperative. But training alone is not the panacea to a facility’s problems. Management’s must be aware that the environment in which their people work, will either progress or obstruct any training that is provided to them. We will refer to our need to address the human element, as the “soft side” of technology. It is estimated that over $60B U.S. is spent on industrial training a year and that only 20% of that training investment is ever applied. RCA has an image problem and needs a public relations agent to reshape its reputation in the healthcare industry! RCA is primarily viewed as a reactive tool. This perception is how we have been conditioned by various regulatory agencies that require us to do RCA under very specific circumstances (usually when something very bad has occurred). When such ‘Sentinel Events’ occur, then we pull the microscope out to take a deeper look using our respective RCA tools. Under this use, RCA is viewed as a ‘Money-Taker’ because it appears only to consume people’s time and resources when they already feel they are overloaded. The majority of times that a Root Cause Analysis (RCA) is ‘required’ is often due to some type of threshold of pain being met. Someone is hurt, we’ve had an environmental excursion, we lost a lot of money in throughput or equipment damage, we violated some regulation, etc. Essentially, when the ‘suits’ show up, something bad has happened and we will do an RCA whether we like it or not. This is the basis of ‘reactive RCA’, where we respond to an incident that has already met certain defined requirements, often called ‘triggers’. [Read more…]
